Nurses from Gateway Health Plan® may call you while you are in the hospital or after you have been discharged.  Gateway is working with a few hospitals and talking with select members who are being discharged to home to be sure services are arranged for you when you leave the hospital.  The program is designed to help you understand what services are planned and to follow the instructions you are given when leaving the hospital.  A nurse will talk with you about making and keeping follow-up doctor appointments, getting your medications, and making sure you received equipment or home care services that your doctor ordered for you. They will answer questions you may have, and help you set up and receive the care you need so you do not need to be readmitted to the hospital.
