Enroll in Gateway Health Plan Medicare Assured^® HMO

Gateway Health Plan Medicare Assured^® HMO is a Medicare approved Special Needs Plan for people with both Medicare and Medicaid (Dual-eligible). To become a member, you must be entitled to Medicare Part A and enrolled in Medicare Part B, receive Medicaid (full or QMB) and live in our service area. Dual-eligible beneficiaries have an on-going Special Election Period, which permits them to enroll and disenroll from Medicare Advantage plans on a month-by-month basis.

Protected Health Information, called PHI, is information that can be used to identify an individual such as name, social security number and birth date. PHI is confidential and protected by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and other privacy laws. In accordance with those laws, Gateway Health Plan Medicare Assured^® HMO may use or disclose a beneficiary’s PHI for payment, treatment and health care operations as described in its Notice of Privacy Practices.   • Fill out the Medicare Assured^® HMO Contact Form and a Gateway representative will call you.
