Pennsylvania Gateway Health Plan^® Disease Management

Gateway Health Plan^® has an ongoing commitment to our members who may be at risk for complications due to chronic medical conditions.  Gateway has implemented the following programs which provide educational information and telephonic Nurse Care Management support to members:

• Asthma - Gateway Health Plan^® has a program for members between the ages of 2 years and 56 years with asthma called AIR Gateway^SM.
• Diabetes - If you are a Gateway member with diabetes, you might want to join the Healthy Returns Diabetes Program.
• Congestive Heart Failure - If you are a Gateway member age 21 or older who has congestive heart failure, or had a heart attack, stents or bypass surgery you might like to join the Help Your Heart Program.
• High Risk Pregnancy - Gateway Health Plan^® has a special program for pregnant women called MOM Matters^® that provides education and support to help you have a healthy pregnancy.

Preventive Health

Keeping our members healthy is the primary goal of Gateway Health Plan^®'s Preventive Health Department.  Members are encouraged to receive health screens, immunizations, proper follow-up care, and to participate in disease management programs through a variety of out reach efforts.
