Complex Case Management

Gateway Health Plan® provides a Complex Case Management program for eligible members. A Care Manager can help you to better understand your health condition and your benefits. A Care Manager can also help to coordinate your health care services by talking with your doctor. The program will tell you about community organizations and resources that may meet your needs.

If you have one of these diagnoses, please call us to speak with a Care Manager:
  • Stage IV Heart Disease
  • Symptomatic HIV/AIDS
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Cancers in active treatment
  • New Traumatic Brain Injury with significant cognitive deficits
  • New spinal cord injury with paralysis
  • Multiple chronic conditions

Participation in this program is voluntary.

Gateway will review the request for enrollment and make the final decision for inclusion in the program.
