Fitness Assured®

Gateway Health Plan® cares about your health and well-being. We want to help you stay healthy, lead an active lifestyle and improve your quality of life. To help you reach your own health and fitness goals, we offer Fitness Assured® – a fitness program created with your needs in mind.

This program is available at no cost to all Gateway Health Plan Medicare Assured® HMO members. As a Medicare Assured® HMO member, you are automatically eligible for this valuable benefit.

Jump-start an active lifestyle.

This valuable fitness benefit can help you get active or maintain an already active lifestyle. It includes:

  • A wide selection of participating fitness centers
  • Full access to all services as standard benefits of membership in participating clubs
  • Articles on healthy living, available by visiting our Fitness Assured® partner website here.
@Home PakGetting started is as easy as 1…2…3!
  • Step One – Talk with your primary care physician to see what level of exercise is right for you.
  • Step Two – Choose from a variety of participating fitness centers in your area by calling 1-877-211-3917 week days from 8:30 a.m. to 5:30 p.m. (TTY users, please call 1-877-440-5580.) You can also search for the fitness centers closest to you by visiting the Fitness Assured® partner website.
  • Step Three – Go to the fitness center and request your club membership through Gateway Health Plan Medicare Assured® HMO. Make sure you have your Medicare Assured® HMO member ID card with you. A staff member will give you a tour of the fitness center, show you how to use the exercise equipment and answer any questions you may have. Don’t miss out on Fitness Assured®! It is another way we’re working to keep our members healthy. Or, if there is no participating network fitness center within 15 miles of your home or you have difficulty getting to a participating network fitness center, an @Home Pak® might be an option for you!
