Gateway Health Plan® cares about your health and well-being. We want to help you stay healthy, lead an active lifestyle and improve your quality of life. To help you reach your own health and fitness goals, we offer Fitness Assured® – a fitness program created with your needs in mind.
As a Gateway Health Plan Medicare Assured® HMO SNP member, you are automatically eligible for this valuable benefit through our contracted provider, Healthways.
This valuable fitness benefit can help you make your lifestyle more active or maintain an already active lifestyle. It includes:
- A wide selection of participating fitness centers
- Full access to all services as standard benefits of membership in participating clubs
- Articles on healthy living, available by visiting our Fitness Assured® partner website here.
Follow these easy steps to utilize your Fitness Assured® benefit
- Talk with your primary care physician to see what level of exercise is right for you.
- Order a Healthways member ID card by contacting the Fitness Assured® toll free customer service number at 877-211-3917 (TTY/TDD call 711). Representatives are available from 8 a.m. to 8 p.m., Monday through Friday, Eastern Time.
- Locate a participating facility in the network by contacting a Healthways
representative at the Fitness Assured® toll-free customer service number at
877-211-3917 (TTY/TDD call 711). Representatives are available from 8 a.m. to 8
p.m., Monday through Friday, Eastern Time. You can also search for the fitness
centers closest to you by visiting the Fitness Assured ® partner website.
- Call a participating fitness center – to schedule a meeting with a
representative, or simply show up and present your Healthways Membership Card at
the front desk of a participating location and tour the location to see all the
amenities you’ll enjoy as a Gateway Health Plan Medicare Assured® member.
