What does this plan cover?
You can click on Benefit Highlights on Gateway Health Plan®'s web site for more details. Also keep and read your Member Handbook as well. It will answer most of your questions. If you need a new Member Handbook or still have questions, please call Member Services at 1-800-392-1147.
How can I get a new Gateway Health Plan® I.D. card for myself or another member if the I.D. card has been lost or stolen?
Please call Member Services at 1-800-392-1147 to ask for a new I.D. card to be sent.
What do I do if the bottom of my card says "No PCP Selected"?
If you already have a doctor (Primary Care Physician) in mind, please call Member Services at 1-800-392-1147 to make your doctor selection.
*If you do not have a Primary Care Physician, please call Member Services at 1-800-392-1147 and a representative will help you find a doctor that takes Gateway.
How can I change the doctor listed on my I.D. card (Primary Care Physician)?
Please call Member Services at 1-800-392-1147 before the 25th of the month so that you can see your new doctor on the 1st day of the next month. If you call after the 25th of the month, you will need to keep seeing your present doctor for the whole next month.
Example:
If you call to change your doctor on January 5th, you can start seeing your new doctor on February 1st.
If you call to change your doctor on January 28th, you will not be able to see your new doctor until March 1st.
How can I find doctors that take the Gateway card? (Examples: General doctors, dentists, foot doctors, eye doctors, pharmacies, etc.)
You can click on Find A Provider link on the Gateway Health Plan®'s web site home page or you can call Member Services at 1-800-392-1147. We can print a list of doctors and mail the list to you or give names of doctors or providers over the phone.
How can I get to my doctor's appointments if I do not have a way to get there?
Medical Assistance offers a program called 'Medical Assistance Transportation Program' or 'MATP'. You must sign up with your local MATP office in order to use this service. Please call Member Services at 1-800-392-1147 to get your local county's MATP phone number.
