Helpful Special Needs Links

Gateway Health Plan's^SM "Special Needs Unit" is designed to provide special assistance to our members who encounter difficulty accessing and/or using medical or human services; understanding how to navigate the managed care system; or learning more about their health care needs or conditions. The unit is staffed by nurses and social workers.

Services provided include the following:

• Link to community resources
• Assist with medication compliance
• Provide education and support for treatment plan adherence
• Assess safety in the home
• Link to MA Transportation Services
• Provide access to the language line
• Link to waiver services
• Assist with coping skills
• Link to Behavioral Health Services
• Assess health risks
• Telephone support for a family in need

Please contact the Special Needs Unit at 1-800-642-3550 Option 1, Monday through Friday, between the hours of 8:30am and 4:30pm to refer one of your patients for any of these services. We look forward to hearing from you.
