Vaccine Updates

Effective January 31, 2007 Gardasil was made available to our female members ages 9-18 through the Vaccine For Children (VFC) Program. Gardasil can be ordered through a Specialty Pharmacy Provider, such as Medmark for our female members ages 19-26. In order to receive the $10.00 administration fee you must bill with CPT code 90649.

Effective January 1, 2007 Rotateq was made available to our members through the Vaccine For Children (VFC) Program. In order to receive the $10.00 administration fee you must bill with CPT code 90680.

Effective January 2, 2007 Gateway reimburses for the vaccine Zostavax (Shingles). You must bill with CPT code 90736 to receive the reimbursement for the vaccine plus the administration fee.
